Oral 7 A Resilience-Based Care Coordination Program Has a Positive and Durable Impact on Health Care Utilization in Inflammatory Bowel Diseases (IBD)

What’s new here and important for clinicians?
Resilience-based care coordination is a strengths-based, positive psychology focused approach to ensuring that the most vulnerable IBD patients thrive despite having been diagnosed with a chronic condition. The GRITT Method at Mount Sinai coordinates care through a biopsychosocial risk stratification tool that drives a personalized, holistic care plan for each patient in our center. This connected approach was superior to treatment as usual, demonstrating 80% or more reductions in unplanned care, namely hospitalizations and ED visits.
What do patients need to know?
Patients should know that they should be seeking out IBD care that considers them from a holistic perspective and that connection and coordination between all stakeholders (Gastroenterologists and advanced practice providers, dietitians, mental health professionals, clinical pharmacists, nurse specialists and caregivers) is critical when their goal is to thrive despite having an IBD.
